Thank you for renewing your commitment to share your pet's love with people who will benefit. Your willingness to reevaluate your team enables Delta Society to assure health care providers, educators, and program administrators that Pet Partners volunteers are highly qualified and will be a valuable asset.

The renewal process involves documenting your team's participation in continuing education, animal-assisted activities/therapy, and community involvement. It also provides an opportunity to reflect on what you have learned about yourself, the people you work with, and your animal teammate. Questions? Call (425) 679-5500.

Follow these steps to complete the renewal process: Renewal Tips

1. Complete the Photo ID form. Don't forget to attach a photo for your new ID badge.
2. Complete the Renewal Application Form.
3. Complete the Volunteer Policies and Procedures Agreement Form. Review your Pet Partners team responsibilities as outlined in the Pet Partners Team Training Manual. Sign and date this form. If you are under 18, a parent or legal guardian must also sign this form.
4. Complete at least 1 of the 3 Team Participation categories. If necessary, continue your responses on additional pages.
   • Continuing Education Summary.
   • Visiting Summary - complete one for each facility you visit.
   • Community Involvement Summary.
5. Complete the Handler’s Questionnaire before the animal is screened for skills and aptitude. Make a copy for the Team Evaluator. This form helps him/her understand your pet’s responses. (Bring this form to the evaluation).
6. Submit a current health screening (include current rabies and fecal) for your animal either on the form provided with this packet or you may attach a signed health form from your veterinarian, which meets the Pet Partners health requirements. Please be sure rabies vaccination and fecal exam are current and included in your packet - failure to do so will delay your renewal.
7. Team Evaluation. Licensed Team Evaluators will have the forms needed to administer the Pet Partners Skills and Aptitude Tests. The Team Evaluator will give you a signed and dated evaluation for you to include in your packet for submission to Delta Society. (Be sure to make a copy for your own records.)    Note:

   Your Renewal Packet must be sent into Delta Society within 90 days of successfully completing your Pet Partners Team Evaluation. When this 90-day time period expires, your are require to retake the Pet Partners Team Evaluation before submitting your registration papers.
